Person-Centered and Trauma-Informed Support
Cornerstone Valley recognizes that each individual has unique experiences, strengths, needs, preferences, and goals. Our services are designed to promote dignity, choice, independence, and meaningful participation in everyday life.
Our approach includes:
- Providing individualized and strengths-based support.
- Recognizing how trauma may affect an individual’s communication, behavior, and response to their environment.
- Using prevention, de-escalation, and positive support strategies whenever possible.
- Including individuals, guardians, families, and support teams in service planning when appropriate.
- Reviewing incidents and support practices to identify opportunities for improvement.
- Reporting suspected abuse, neglect, or exploitation in accordance with Oregon law and organizational requirements.
Safeguarding Interventions
Cornerstone Valley uses the least restrictive interventions reasonably available to protect the health and safety of the individuals we support and those around them.
Safeguarding interventions are not used as punishment, discipline, retaliation, or for staff convenience. They may only be used when an individual’s actions create an immediate risk of serious injury and less restrictive interventions have not been effective or cannot be safely used.
Cornerstone Valley works to prevent and reduce emergency restraint use through:
- Individualized behavior and safety planning.
- Staff training in prevention and de-escalation.
- Identification of triggers, warning signs, and effective calming strategies.
- Ongoing review of incidents and documentation.
- Collaboration with individuals, guardians, families, clinical professionals, and other members of the support team.
- Follow-up actions intended to reduce the likelihood of similar incidents.
